I didn't run an e-store but we still had a similar set up for my previous startup so here are my answers to your questions:
1. We programmed our own so I can't really help you here in regards to existing solutions as it was better for us to code our own solution.
2. We had a merchant processing account but I can't really recommend any one account as you should shop around and try to find the best solution for your business. Rates are negotiable and depending on the volume your business can do, you can renegotiate pricing. For us, we found a good solution where our merchant account was tied with our bank and owned their own gateway (if you have no clue on the terminology I'm referring to, please research this) and since everything was through one provider, it made things much easier and they were willing to match all the other competitors we found rates through which ended up being the most ideal solution. To top it off, we had a local agent assigned where I had direct access via the agent's cell number 24/7 which made things amazing should I need to get in touch with someone. Again, depending on where you're located, you're best to try and shop around and find the terms that best fit your business.
3. Hosting will also vary greatly depending on the level of traffic you have and will most likely change over time as your business grows. Starting out, I recommend running with a shared hosting service (I recommend Bluehost), then switch to managed hosting as your business grows (I recommend SingleHop) and then finally switch to a cloud service for anything beyond this (Amazon EC2).

I am using the Shopp plugin for my Wordpress store. If I had a dedicated server I'd probably be using Magento instead as a separate ecommerce solution in addition to my wordpress blog
