resale certificates

Hi
I sold some of my products to a store at wholesale price and the store is within the same state as my business. So, I asked him if he has a resale certificates and he said he ran out of copies and he told me to get an empty resale certificate and he will sign it. Btw, the State is NY. Where can i get empty certificate?

If you go to the board of equalization website, you can usually get resale certificates. You can get a copy of his license, but (at least here in California) you must have a signed statement which states he is buying the product for resale.

The resale cert is the same place you are paying sales tax to - you ARE paying your sales tax right (I got audited in 06)?
