None Disclosure Agreements- Who is to say what information was disclosed?

Hello, I've recently been researching NDAs and looked up some sample NDA templates on the web. Interestingly, none of them included a part on exactly what type of information has been disclosed?

For instance, say I have invented a new software concept. A non-disclosure agreement is signed by both parties. Then the guy turns around and sells the idea or uses it to get promoted at his current job. I try to go after him for disclosing the information we discussed, but where in the contract will there actually be a record of what information has been disclosed?

Could anyone shine some light on this for me. Any help would be very much appreciated.
