Need Disclaimer and Agreements Professionally Written

Hi guys,

I own a new mobile repair business, and I need a firm or business that can create a professionally written disclaimer and agreement that I have my customers sign before I begin any work or even enter their household. It is an effort I'm making to try and prevent people trying to screw me out of money or fraudulently requesting refunds, as well as to make the customer feel more confident in the service I'm providing via stating my warranties and other information that is "on their side".

I want it very easy to read, point by point, and I do not feel qualified or versed enough in this to do it myself. Any suggestions? I'd prefer an online resource where I can just pay via credit card, receive my paperwork, and be done. Thanks for the help in advance!
