Change Name of LLC

I would like to change the name of my registered LLC.

I know there is a state filing fee. I don't have to get a new EIN (to my knowledge, because I am only changing the name and nothing else).

What I would like to know is, is there a form for this or do I just write a letter?

If it's just a letter, what does it need to contain, aside from the obvious (new/old biz name, EIN, address, etc)?

I am in the state of Georgia, if it makes a difference.
