When a t-shirt company puts out a design/shirt does the design on the shirt need to be copyrighted?
Also, I want to use a cereal company character, like Tucan Sam, on one of the designs/shirts. ..do I need permission or anything?

Yes. if you are using someone elses picture, even to copy one off the internet, you need permission.

I said yes. You have to seek permission (legally) if you're using whether a personal image or a company logo.

And also, you don't have to Copyright (it would actually be trademark, not copyright), when ever you create your own logo. You technically own the Trademark and can place TM next to the logo (but you have no solid proof of ownership of the logo). If you wish to use the Registered 'R' (®), you have to register with the US Patent and Trademark Office (United States Patent and Trademark Office Home Page).
You can also double up and copyright a graphic, but with a trademark registration it should be redundant.
