doesn't that response-o-matic go to an ad if you're using the free version?

it should be rather simple to have a form that submits the email. look online for a free script to do it.

if you're going to pay someone it shouldn't take them more then 5 minutes to do it.

Typically it doesn't take long but it depends on the host and plan you are using. Sometimes, some research and trial and error are involved to figure out what you can use to send the mail. You will need to know if it is using a version of ASP SendMail and your mail server details.
