Need ideas for an online business survey

I want to put a survey on my blog that will ask questions about the problems people face when trying to start a business and/or make money online. I want the survey to be multiple choice and have a maximum of 10 questions. I was wondering if any YE'rs have any ideas?

I'm not necessarily asking this questions right now. I'm trying to develop a good list of questions to put on my blog. I plan on developing my product offerings from this survey.

What is the biggest problem you have getting people to your website
    - Don't understand SEO
    - Don't know how to set-up PPC

What is the biggest problem you have starting a company
    - No Money
    - Don't know the first steps
    - Don't know how to write a business plan
    - I'm scared to take the risk

How do you do your planning
    - I just go with my idea
    - I write down some general ideas as a guide
    - I write a basic business plan
    - I write a full professional business plan

Have you tried buying programs that say you will make money doing a certain task?
    - Yes
    - No

Have you tried a MLM Program (Multi Level Marketing)
    - Yes
    - No

How many hours a week do you put into your online business
    - 0 to 5
    - 5 to 10
    - 10 to 20
    - 20 or more

If you have an online business what type of site are you running?
    - Content: making money from advertising
    - Product: making money from sales
    - Affiliate Site
    - I do not have a site
    - Other (please list)

    Do you read eBooks
    - Yes
    - No

If you are thinking of starting a business what is your budget
    - Under $250
    - $250 to $1000
    - $1000 to $2500
    - Over $2500
    - Not thinking of starting a business

I'm trying to do research on what people are thinking when it comes to starting their own business. I'm looking for choke points that scares people from moving forward with their dreams.

Any input you all might have would be helpful!

Hmm...sounds like an interesting survey to say the least. One of the things that I would suggest adding is perhaps questions related to passion, drive and ambition. What turns entrepreneurs on when they first realize their idea is worth something and they love doing it.

Make the survey meaningful to your audience. Leave a message at the end that might be made depending upon the answers. Such as whether your and your business are compatible or not. Will you be successful or not? Something that they get as a result (like a receipt at the cashier) of their efforts. It will generation more attention.

    Good luck!
