    Adobe Professional has a very nice feedback form that you can customize. You can ad it as a downloadable link for your visitors to fill out or,.. After they use your service, you can email it to them and ask for feedback. There are a few other ways that are easy,. Depends on what and how you want to get the info.

    ^Do you mean comment form? you can edit or add a code using adobe professional as what tytorelli said. Or embed a third party comment form.
