Are all websites required to be under a business entity either as itself or a DBA?
No. We would automatically make the assumption in the USA that this is a "Sole Proprietership" if someone just buys a domain and starts making money with it.
Many online business owners do not register and pay personal income tax. As long as you declare your earning properly, things would be fine.
You definitely do not have to create business/corporation to own a website, build one, promote it, etc... If you are generating revenue from the website I would certainly recommend creating an LLC or at least some corporation instead of filing the income under your social security number.
